To Register:
1. Complete the online registration by February 28, 2025

**Registration will be held on Friday, March 7th by appointment only at the Charlotte Lappla Elementary School.
Children must be 5 years of age before September 1, 2025.

Vaccination Requirements:
4 Doses of DTaP (Diphtheria, Tetnus, Acellular Pertussiss - 1 does after 4 yrs. old)
4 Doses of Polio- 1 dose given on/or after 4yrs. old
3 Doses of Hepatitis B
2 Doses MMR (Measles, Mumps, Rubella)
2 Doses VAR- Varicella (Chickenpox)

Proof of residency in the Wellsboro Area School District. List of acceptable proof of residency (no PO Boxes and no Driver’s License) is one of the following:
Lease
Current Utility Bill
Current Car Insurance
Current Car Registration

Custody Papers (if applicable)

Foster papers (if applicable)
